Casey is very ill-

Not sure where to begin...
We all had very long morning- Casey does have a blood clot in her arm. There could be more elsewhere... (Not crossing that bridge).
This clot in her arm? I got the impression that it was small?
(Admittedly, I do not know what the definition is of 'small' for a clot either).
Maybe this is small?
They gave her an ultrasound-
Should anyone know what the heck this represents, please let me know? hehehe
The pain she described as hot and stabbing- "like there's a knife in there and it won't come out."
She felt it all the way down to her thumb- said it felt like she banged her elbow- Just perpetually.
She is now also on Lovenox, 50/5
Not sure, but I still have to look all that info up.
(I do remember giving these shots to Randall in his tummy after his stroke).
She gave herself one injection at the hospital-
Another later tonight-
We got home, trying and trying to get through to Dr. Sutherland-
His asst called me back shortly after 4. Dr S. wants Casey there by 10 tomorrow morning.
Ann-Marie told me that this sounds very serious; and could not understand why her docs here wanted just to give her more infusions.
(Hey wait! Wasn't that MY question)!!!
She did offer to check on that for me- This offer, right before she said to come in by 10AM - So I declined that offer- Knowing that we should have some answers, or at least maybe a better "plan" after we see him.
Good timing here- My dryer has just buzzed...
That stated, I need to fold my clothes, treat my freakin' head, and lie down and 'nap' for a bit. I will pack when I wake.
We will leave here by 3AM. With the good lord willin' and the creeks don't rise.

ReplyDeleteThey travel through your body blocking arteries...I'm glad Casey is on Lovenox. It should keep her blood thin.
Infusions at this time, I think, would be disastrous. After my first PE I was on Coumadin for about a year, then taken off. After the second, I'll be on it all my life because of my 'history.'
It was caused by estrogen, (birth control), which I stayed off until a GYN convinced me it was okay. Second blood clot soon after.
A blood clot is a blood clot. Period.

ReplyDeleteI'm still wondering about all the apparatus in the picture for the Lovenox injection? Mine wasn't at all like that. It was a small syringe holding the proper amount, & the needle was as thin as a thread. This thing looks like a horror movie.
Did any doctor tell you that when on a blood thinner, contusions, sprains, cuts take a very long time to heal, & infections occur more frequently? This last time I went to the ER for an acute sprained lower leg, when they discovered I also had a PE., because of the extremely high dosages of both Lovenox & Coumadin to dissolve the blood clot, (as the next ER doctor told me), prevented healing of the sprain. It turned into a hematoma where blood vessels 'broke' into a wound that hemorraged & was so painful I actually passed out before dialing 911.
PLEASE ensure that Casey is very careful about any potential injuries?
Another problem with blood thinners is if you are having an unknown bleed somewhere that will get worse oh my what a predicament ..
ReplyDeleteYES. Also when surgery needed, even dental cleaning! The INR needs to be stabilized, but can change week to week.

What's the priority here? Get rid of the blood clot, FIND THE SOURCE OF THE PROBLEM. How difficult is that, I wonder?

Yes, so far as I've seen. They aren't getting to the root of the problem & only want to do what they did before that didn't work.
ReplyDeleteFor a short while, a VN came to take my prothrombin The results were immediate. I'm glad you mentioned this. It's possible that my insurance would pay for that 'equipment', because there are many times I can't make it to the lab. Sigh. My doctor would want to see me to approve it, & if I can't go to the lab, how can I go to see her.......

ReplyDeletei would ask if she has a history of third spacing?Igot Lupus anticoagulant antibodies from it and I had spontaneous clots in my lungs. there i s a test for it .I guess i would ask for the test..It goes along with pancreatitis.if she does it can become active at any time, at least that was what I was told.Will of course pray for her. nothing more scary than a clot and a small one at that.
ReplyDeleteCasey is going thru what Drew went thru back the end of June/ first of July when they put that first PICC line in his right arm. The Doppler scan is good for detecting the blood clots...I can't really see the "doppler" picture well enough to read it..and I always like to listen as they are doing it to the "blood swishing" thru the veins as I watch the pictures on the screen.
ReplyDeleteWhere Casey is showing her arm and the area...yes it will hurt all the way up and down that vein..and it does feel like a knife stabbing..and burning. Warmth to the skin in the area and red streaks are signs of a blood clot as well as swelling at times(Drew currently has a clot in each calf..despite being on Lovenox). She could have "phlebitis" (inflamation of the vein) as well. Have they said if this is a DVT(deep vein) or phlebitis(possible superficial) ? Either way..blood thinners would be the course.
Drew's Lovenox comes in a pre-loaded syringe..(he is currently on 80mg twice a day). Between that and his insulin shots...his tummy looks like a pin cushion. The hospitals sometime use "fragmin" (another low molecular weight heparin) while they are in the hospital(because of insurances,etc..) thats why I think it looks like Casey is using a regular syringe to inject herself there in the bottom picture.
Lovenox doesage is based on paitents weight. I look for them to up Drew's doseage as he has gain some weight since coming home.
While using any "blood thinning" therapy....be it asprin, Coumadin, Lovenox, etc....the paitent must ALWAYS inform any medical person (Dr, Dentist...etc) that they are using a blood thinner..before any procedure. The risks of bleeding while on blood thinners increases...as well as bruising...and care must be taken not to get bruised or when using sharp objects not to get cut.
The use of Lovenox over Coumadin is in case of Casey needing surgery...they won't have to wait for in case she's too thin (they will still get the PT/INR results to know the "thinness" of her blood)....because Lovenox doesn't require the same regimine of having frequent PT/INR's done to check the Coumadin levels. Lovenox once a desied level is reached stays at that level..unlike Coumadin (Drew was never ..in the 6yrs he took Coumadin/Warfin...able to be therapuetic...one of the reasons they have discontinued it in him). Coumadin requires constant checking to "regulate" to a therapuetic level.
While on blood thinners...they should give ya'll information on what "foods" interfere with blood thinners....and what medications...over the counter as well as prescribed medications(thats why all medical personel has to know when you are taking blood thinners). As far as foods...it's the "Vitiamin K" that you are looking out for..as it is a natural clotting substance. It's in alot of the green leafy vegetables. We were told that if Drew wanted to eat these certain foods....he had to eat the same amount "regualarly..and consistantly" so they wouldn't interfere with his blood thinners. (Drew doesn't do anything "green" so its not that big an issue here..lol)
We will keep you all in our prayers. We know what it is to have to deal with the blood clotting issues and all that goes along with dealing with blood clots. Clots no matter the size..or place in the body..are serious. I'm happy she's going to see Dr. Southerland today. I know ya'll will get good information from him and his staff. Ya'll drive safe..take care. Keep us informed...hugs for all of you.
P.S. Tell Casey (according to Drew) ...unfortunately you still turn into a "pin cushion"...as you can never escape "blood work" but there is no "IV" needed for a Lovenox injection. Since he knows she has had as many IV's as he's had to have while in the hospital.

Either way, by now she is firmly in Dr. S's care and I am sure Anne will post updates as soon as she is able.

Dr. S. has ordered many tests-
We will have to return for a few of those, (and for an appointment at the hematology part of the hospital).
They drew about 12 vials of blood-
Casey is ordered OFF the lovenox for now. (He believes that most blood clots will resolve). For follow up to go back to the hospital, or to her GP in town. Will keep an eye on it.
But he does want to investigate WHERE this is coming from? More accurately put, where her iron is going to.
He also said that she probably will not need the infusions- A few Rx things to change, and to try those.
She is also to take Protonix once a day.

We all know just how dangerous a blood clot can be Anna, when I saw the heading a day ago I wasn't sure if I really wanted to come here to learn the know the outcome :-(

Glad she's off the Lovenox (words we wish we could hear..and hope to in 6 months to a year if we are lucky and the NEW oral meds come when they say they will). Heaven help they took enough blood didn't they? Drew's last blood draw was 9 vials.
ReplyDeleteGlad he's also going to "look" to where the previous iron infusions went...instead of just pumping in more. Something is going on thats for sure...and it looks like Dr S is going to investigate thoroughly.
