I had hoped never to post another sad blog about Casey being sick.
Caseys temp is up 2 degrees.
Her b/p seems to be normal.
As of yesterday (12-31-08) she was doing so good that they have now turned the dilaudid drip down-
(A mere 30% of what she was getting for pain).
Yesterday she was given a full menu. YAY! And even got to eat supper!
Her blood sugar (in her blood draw) was down to 46 this morning... then shot up to 168 within 20 minutes. (Without her having eaten anything).
Today she could not eat without falling to sleep- not so much the pain.
She is turning a golden color once more. (I see that anyway).
She is mighty incoherent.
I had the nurse put a call in for the doctor within 5 minutes of me being there.
It is a Holiday. Where in the hell are the doctors???
I am worried all over again...

As you can see, her legs and feet are also quite swollen. (They have given her diuretics twice in 2 days for this already).
WTF?

Now if the nurse could actually GET permission from the damned doctors, they might even be able to get that done.
I do not think I am over-reacting. (Maybe just a little scared)...
I took her pain button away. She has not pushed it all morning, nor has she asked for it back.
If there is a problem with the meds in that vial, they would leave her this way, (I think with respect to the incoherence anyway).
Please, if you will, a few more of those wonderful prayers?
I gotta go raise hell.

Well, I'm glad she got to eat something. Sorry to hear that things aren't going so well today. Please keep us posted and will send up more and more prayers. Tell Miss Casey that if she doesn't hurry up and get better, I'll have to send her another monkey butt. Bet that will put a smile on her face. Tell her to be watching her mail. Will send something to put a smile on her face......

Oh Anne this has me worried again too. And there around the hook end of her incision, does that look as if it is becoming distended? When her tummy was filling with blood did it start in that area?
ReplyDeleteThe swelling really concerns me. Fluids of some kind are definitely being retained. And the sugar count going up without eating anything doesn't sound good either. That alone is going to be a challenge to figure out.
And you have every reason to be worried sick. We are all worried about our little Miss Caseyface! Know that you are all in our prayers!

when Caseys sugar was down to 46 the nurse could have given her an insulin to make it go up , maybe you could ask her if she did, because she could have given her too much. but as deeannes mentioned yes uncontrolled blood sugars play havoc with your feet and legs swelling and aching. Gosh Anne I was so hoping and praying this nightmare would end finally. You get a Dr. there, dont stop until one comes. and Anne you got to take care of you too, More prayers go out for Casey and you hon

I can't offer much but I can tell you this:
ReplyDeleteRandall is diabetic and when his blood sugar runs REALLY high, he can become incoherent and sleepy. There have even been instances where he passes out and awakens a minute later... He has trouble with his legs.
Some of what Casey is going through today can be attributed to her blood sugar... The rest I'm not sure.
Seeing her belly swelling up again is a worry... I KNOW you gave 'em hell til you got a doctor there...

ReplyDeleteEvenin Rock, Im hoping you raised some hell and got some help for that girl. The other advice you have gotten is correct about the sugar giving all those symptons.. Until they get the meds right for her diabetes.. you may continue to see these kinds of things.
ReplyDeleteI was once told that the meds for db can be different for each person. So that what is right for you with the same kind of db may not be right for her. Im sure it will take awhile for them to get that right.. and those doctors need to be seeing her everyday.. and not just occasionally.. just my opinion mind you and you know how I am about sharin..hehe
the temps and the jaundice.. I dont think are a part of that though..
Of course, what do I know.. I argued with my doctor when she told me I was diabetic.
Hopefully shes doing better now and youve been able to talk with someone who has been there to help her.
My prayers will still continue for her as she continues to heal and for you as you continue to worry and fight for her.

ReplyDeleteI came back here, ate some dinner and promptly fell asleep.
My apologies.
As for Casey...
She is back on her feet. AND coherent.
I think I must have made enough noise because shortly after I had posted this her staples started bleeding and weeping.
The nurse asked if I would like that blood draw now?
I said "yes"
In about 15 minutes there were two (not one, but TWO) surgeons in her room.
Then the phlebotomist girl too.
Her blood tests showed that her hemaglobin was at 12.1, down just a few .s from the AM test.
And yesterdays readings were so low. (I was the only one that caught that too)?
Wrong above about them giving her one unit of blood. They gave her two yesterday.
Her white count is still quite high. 20.1 (but down from 56.4 days ago).
Her PTT ...
(don't ask me what it stands for, to do with whether she is getting enough/too much heparin) is at 26. And that is normal.
The doctors said the swelling in her legs and feet has much to do with her liver and not anything CHF or otherwise heart related. He explained it all quite technically, so we'll just take his word for it.
They also said 'some' leaking and/or bleeding from her staples is normal.
Scary, but normal.
He also related that these problems could be transfusion related. Not that she didn't get the correct blood TYPE, but sometimes our bodies make an effort to not like the new blood cells.
She was on her feet later and even went up to the cafeteria with me when I got my dinner. (Too much turkey is why I fell asleep)...
I left her in her room. She was going to go to Theresa's room.
Either there or walking the halls is where I find her if she is not in her own place.
Theresa had the same surgery Casey did-
Only had a rougher time. She has been there 3 months already.
I think I will try to get a bit more sleep.

Even the doctors agreed that they could not tell me how come her blood sugar went weird, except to say that it might have been because of the TPN. (She only gets that at night now). But they had removed the bag when her counts were down...

They had given her nothing at that point.
ReplyDeleteThey said they would redo the recipe for her TPN for last night and see if they could get that stabilized for today.
But even doing that is going to be a lot of guesswork. He thought perhaps a few of those transplanted Islet Cells are starting to produce... (Fingers crossed)?
My thoughts about the swelling was that if she was walking 'x' miles a day (and she surely does) that there would be no swelling.
See what little I know?
See once it begain weeping and bleeding my own "hell hath no fury" was little compared to her nurses.

They gave ger a huge gauze pad when they saw how soaked her nightshirt was.
But by that time it had slowed enough to only leave about a 50cent size blob on that.
She was SO out of it that it scared me bigtime.

I am hoping the noise maker is otherwise harmless, but BE careful!
It's great to hear that even with the rollercoaster Casey is getting around and even has a friend to identify with! I am sure it makes it easier on Theresa too.

AND the nurse had also cut the amount down a 2nd time after I had given her notice of dilaudid being recalled from TWO companies, due to overmedicating, high potency, with them. (Recalled were the pills -not the mix, but one can never be too careful).
The doctors do come in every day. Last friday though it was early, VERY early, in the AM, and then we all waited until LATE saturday afternoon to see another MD face around.
I went down to the front desk again before I came back to the room. A new guy on now, and the night one hadn't relayed the message either.

Hospital people, in my own little opinion, really shouldn't be drinking and pounding on doors and whining out in the hallway in the middle of the night.

I had a surgical procedure done a decade or so ago, after the surgery the surgeon put a drainage tube in the incision site to help any fluid drain away instead of building up, it also helped them monitor the color of the fluid for any blood, I wore this tube and bag for about a week. So my question is - why can't they do that for Casey?
Sorry to hear about your noisy neighbors. I deal with this issue at the hotel all the time. Usually I call the noisy room from the front desk and ask them to bring the noise level down (after 11PM - noise level down to zero), I wait about 5 minutes and then take a walk to their room and stand in the hallway, outside their door, and listen, if I still hear too much noise I knock on the door and warn them that if I hear any more noise coming from their room I'll evict them (with police escort). That usually does the trick, but have had to bounce quite a few in the last 4 years. The tricky part is to evict the noisy guests without making even more noise and disturbing the other guests even more. Whatever you do don't ever confront the other guest yourself. It's usually hotel policy for employees to never tell the noisy guest who has complained - you can imagine what that can lead to.

GOOD MORNING!!! Just checking in to see if there was an update, and, joila, there it is!! Happy to hear that Casey was back on her feet again. That is always a good sign! Also happy to hear that the doctor said that alot of what's going on is normal but scarey. Just know that I will continue to keep ya'll in my thoughts and prayers. Keep us posted and get some rest yourself. I'm glad you had a bite to eat and got some sleep last night. Don't ever apologize for falling asleep. You needed the rest and your body took over.

They put the tube in for my gallbladder too.
ReplyDeleteMany doctors do not care to use them- Generally a body 'should' do away with it all anyway...
But there are so many infections, especially at hospitals, that they don't use them (much) anymore.
Dr. Sutherland is one that prefers NOT to use them unless things are very critical. (Her surgeons on Sunday were then bound by his preferences).
So you mean I should not have opened my door and said anything?
I stood full in the doorway and said, "Can you kepp it down please?"
The lady that was whining begging and pounding on the damned door turned around with attitude, (and quite PROBABLY would have said something else), but then Punk barked and growled.
hehehe She looked up at me again and said, "oh sorry."
The 2nd time I just said "please?"
I did ask the guy at the front desk if they could be moved to another hotel though. ;-)

It's always best to let the hotel employees handle it, we are trained to do that. BTW I've had a fight break out because of guests confronting each other, (I kicked them both out!) so the best thing to do is just stay behind your closed door and call the front desk.

ReplyDeleteI only just got up about a half hour ago. I was on here late and then got into watching a movie during the wee hours of the morning. Been reading all the updates and am glad to hear Casey is doing good today. Hard not to worry in situations like this. I know it takes time to heal but it's those bad days mixed in with the good ones that I don't like. I have read somewhere that we are given the bad days so that we may appreciate the good ones. Enough with the bad days already! We get it!

I think I speak for us all when I say that NONE of us like the scary parts.
they would have still had to reopen her to stop the bleeding. They stopped using those tubes after finding that they were one of the huge causes of postoperative infections. I can tell you that my mom had a tube on her first mastectomy and almost died from a staph infection. The second time the doctor declined the tube and she had no problems with infection at all. That was enough to convince me that the doctors might be on to something. an open wound is a risky one.
ReplyDeleteJust to clarify, insulin will make her blood sugars go DOWN!!!!! If her blood sugars were 46, she needed sugar at that point, juice or whatever. What you saw with the rise of her blood sugar is called a sugar dump - her beta cells (origonally in her pancreas, now in her liver) sent a message that the blood sugars were low, and the message was accurately received by the liver which then did a sugar dump, the way things are supposed to work!!!! This is indeed a good sign! I would think that it would not be unusual that while this is all getting going and the cells are learning to work again, that they would work in a less than ideal way. And yes, when she is low, below 70 or 80, she will feel awful - dizzy, sweaty, hot or cold, on and on. And when she does a fast rise like that she may also feel cruddy. Kiley does, though for her the changes are a hundred points or more. So, just to clarify, so you know, insulin makes blood sugar go down (not up!), sugar makes it go up. Insulin is the bridge that takes sugar from your bloodstream into the cells. Hope that helps.

They don't usually like it BUT if it means you understanding better what the hell they're saying they should do it with no problem...
(I had to do this several times when my step-dad had triple by-pass heart surgery. The doctors HATED me LOL)

When I got back from the hospital, I did make sure day shift told the manager- He had just gotten in- a real nice young man too. Then I tried to rest. (Didn't work). So at least all the shifts know.
In defense though I did not go out into the hallway. Had she lipped off, I would have closed my door and then called downstairs.
In reality I really am quiet as a mouse. (Unless said mouse is in Sputtys mouth)...
I don't MAKE trouble, but... (you fill in the ...).

they have to do a scan (U/S?) of her arm where her PICC line is, as they believe she may have a blood clot there now...
When she'd called me this morning I was almost afraid to answer the phone.
ReplyDeletei asked how she felt, and she said "perfect" and she was sitting at the end of the hall in the big old wooden rocking chair soaking up the sunshine.
Now that's what I like to hear!
